Drug-Induced Photosensitivity

DRUG-INDUCED PHOTOSENSITIVITY

Drug-induced photosensitivity: cutaneous adverse events due to exposure to a drug and either ultraviolet (UV) or visible radiation. Reactions can be classified as either photoallergic or phototoxic drug eruptions, though distinguishing between the two reactions can be difficult and usually does not affect management.

The following criteria must be met to be considered as a photosensitive drug eruption:

• Occurs only in the context of radiation

• Drug or one of its metabolites must be present in the skin at the time of exposure to radiation

• Drug and/or its metabolites must be able to absorb either visible or UV radiation

PREVENTION AND MANAGEMENT

• Caution patients of the potential reaction for drugs considered to be potent photosensitizers; monitor.

• Emphasize sun avoidance and sun protection upon treatment initiation.

• Discontinue offending drug once diagnosis of drug-induced photosensitivity is made. Implement secondary preventive measures (eg, sun avoidance esp. during peak daylight hours, use of sun protective clothing and sunscreens with both UVA and UVB protection) if drug discontinuation is not possible.

• Administer medication in the evening if appropriate.

• Use of topical or systemic corticosteroids may be helpful to treat drug-induced photosensitive eruptions in symptomatic patients.

FDA Approves First Molecular Test to Screen Blood Donors for Malaria https://www.empr.com/home/news/fda-approves-first-molecular-test-to-screen-blood-donors-for-malaria/ Tue, 26 Mar 2024 19:40:00 +0000 https://www.empr.com/?p=217822 The Food and Drug Administration (FDA) has approved the cobas® Malaria, a qualitative PCR test, for use on the cobas® 6800/8800 systems to detect Plasmodium (P. falciparum, P. malariae, P. vivax, P. ovale and P. knowlesi) DNA and RNA in whole blood samples from individual human donors, including donors of whole blood and blood components, as well as other living donors.

The cobas Malaria test is also intended for use in testing whole blood samples to screen organ and tissue donors when samples are obtained while the donor’s heart is still beating. The test is not intended for use: as an aid in diagnosis of Plasmodium infection; on samples of cord blood; or on cadaveric blood specimens. 

The approval was based on data from performance studies which showed that the cobas Malaria test demonstrated 100% clinical sensitivity for P. falciparum, P. malariae, P. vivax, P. ovale and P. knowlesi. Moreover, the test demonstrated 100% clinical specificity in a zero-prevalence population.

“As the first FDA-approved blood screening test for malaria, this represents an important step forward in safeguarding the global supply of donated blood,” said Matt Sause, CEO of Roche Diagnostics. “The approval of cobas Malaria represents a significant advancement in malaria detection, offering health care professionals a reliable tool for donor screening and improving the safety of patients worldwide.”

The cobas Malaria test will be available at the end of the second quarter of 2024.

HealthDay News — There has been another case of locally acquired malaria in the US, this time in Maryland, authorities report.

This case is the first locally acquired case, not related to travel, in Maryland in more than 40 years. Seven locally acquired infections in Florida and 1 in Texas have all been reported since May, but no evidence suggests the cases in these 2 states are related, according to the CDC. Health officials said the Maryland patient had not traveled to those 2 states.

“We are taking this very seriously and will work with local and federal health officials to investigate this case,” Maryland Health Department Secretary Laura Herrera Scott said in a statement.

The state health department said the patient, who is hospitalized and recovering, has not traveled outside the US recently. The patient had gone to the hospital with symptoms including fever and sweating, said David Blythe, MD, director of the health department’s infectious disease division, NBC News reported. A mosquito may have acquired malaria from a person who had acquired it while traveling and then spread it via a bite to the Maryland patient, Blythe theorized.

Malaria, a mosquito-borne illness, is rare in the US and was considered eliminated nationally by 1951. Death from the disease can typically be prevented with treatment, the CDC said. Nearly all of the more than 2000 malaria cases reported annually in the US are acquired by travelers overseas.
