Mechanism of Action

Ephedrine sulfate acts directly as an agonist at alpha- and beta-adrenergic receptors and indirectly causes the release of norepinephrine from sympathetic neurons. Pressor effects by direct alpha- and beta-adrenergic receptor activation are mediated by increases in arterial pressures, cardiac output, and peripheral resistance. Ephedrine sulfate stimulates heart rate and cardiac output and variably increases peripheral resistance, resulting in increased blood pressure.

Indications

Treatment of clinically important hypotension occurring in the setting of anesthesia.

Emerphed Dosage and Administration

Adult

Do not dilute. Give as IV bolus. Initially 5–10mg, may repeat as needed; max total dose: 50mg.

Tolerance may develop. Repeated doses may cause tachyphylaxis. Have alternative pressor agent readily available. Hypertension. Renal impairment: monitor. Elderly. Pregnancy. Nursing mothers.

Interactions

Severe postpartum hypertension with concomitant oxytocics (eg, methylergonovine, ergonovine); monitor. Potentiated by MAOIs, clonidine, propofol, atropine; monitor. Antagonized by alpha-blockers, beta-blockers, reserpine, quinidine, mephentermine; monitor. May inhibit hypotensive effect of guanethidine; monitor and adjust dose. May reduce onset time of neuromuscular blockade when used for intubation with rocuronium if used simultaneously. May decrease efficacy of epidural blockade; monitor. Concomitant theophylline may increase frequency of nausea, nervousness, and insomnia. Concomitant digitalis glycosides may cause cardiac arrhythmias; monitor.
