Indications

Treatment of clinically important hypotension occurring in the setting of anesthesia.

Akovaz Dosage and Administration

Adult

Give as IV bolus. Initially 5–10mg, may repeat as needed; max total dose: 50mg. Adjust dosage based on blood pressure goal.

Tolerance may develop. Repeated doses may cause tachyphylaxis. Have alternative pressor agent readily available. Hypertension. Renal impairment: monitor. Elderly. Pregnancy. Nursing mothers.

Interactions

Severe postpartum hypertension with concomitant oxytocics (eg, methylergonovine, ergonovine); monitor. Potentiated by MAOIs, clonidine, propofol, atropine; monitor. Antagonized by alpha-blockers, beta-blockers, reserpine, quinidine, mephentermine; monitor. May inhibit hypotensive effect of guanethidine; monitor and adjust dose. May reduce onset time of neuromuscular blockade when used for intubation with rocuronium if used simultaneously. May decrease efficacy of epidural blocker; monitor. Concomitant theophylline may increase frequency of nausea, nervousness, insomnia. Concomitant digitalis glycosides may cause cardiac arrhythmias; monitor.
