Indications

Postherpetic neuralgia.

Neurontin Dosage and Administration

Adult

300mg once on day 1, twice daily on day 2, and 3 times daily on day 3; may titrate up to usual max 1.8g/day in 3 divided doses (doses up to 3.6g/day have been used without added benefit). Renal dysfunction: CrCl 30–59mL/min: 400–1400mg/day in 2 divided doses; CrCl 15–29mL/min: 200–700mg once daily; CrCl ≤15mL/min: 100–300mg once daily (see full labeling); hemodialysis: 125–350mg after session.

Warnings/Precautions

Monitor for DRESS/multiorgan hypersensitivity, anaphylaxis, angioedema; discontinue if occurs. Increased risk of suicidal thoughts/behavior. Monitor for worsening of depression, suicidal thoughts/behavior, and/or unusual changes in mood/behavior. Underlying respiratory depression; monitor and consider initiating at a low dose. Avoid abrupt cessation. Renal impairment. Elderly. Pregnancy. Nursing mothers.

Interactions

Increased risk for serious respiratory depression and sedation when concomitant CNS depressants, opioids (eg, morphine, hydrocodone, oxycodone, buprenorphine). Give 2hrs after antacids. May antagonize hydrocodone. May interfere with some urine protein tests (eg, Multistix-SG).

Adjunct in partial seizures.

Neurontin Dosage and Administration

Adult

Add or remove other AEDs over at least 1 week. Initially 300mg three times daily. Usual range: 900–1800mg/day in 3 divided doses; max 2.4g/day (up to 3.6g/day has been used short-term; see full labeling); max 12 hrs between doses. Renal dysfunction: CrCl 30–59mL/min: 400–1400mg/day in 2 divided doses; CrCl 15–29mL/min: 200–700mg once daily; CrCl ≤15mL/min: 100–300mg once daily (see full labeling); hemodialysis: 125–350mg after session.

Children

<3yrs: not established. Give in 3 divided doses; max 12 hrs between doses; titrate over 3 days. 3–11yrs: initially 10–15mg/kg per day. 3–4yrs: titrate to 40mg/kg per day. ≥5yrs: titrate to 25–35mg/kg per day. Max 50mg/kg per day. Renal impairment: not studied.

Somnolence, dizziness, ataxia, fatigue, nystagmus; diplopia, amblyopia, tremor, dysarthria, amnesia, back pain; children also: viral infection, fever, nausea, vomiting, emotional lability, thought disorder, hyperkinesia, hostility.
