Storage

Store between 20°C to 25°C (68°F to 77°F); excursions permitted to 15°C to 30°C (59°F to 86°F). Protect from freezing.

Mechanism of Action

Nitrofurantoin is reduced by a wide range of enzymes including bacterial flavoproteins to reactive intermediates which are damaging to macromolecules such as DNA and proteins.

Furadantin Indications

Indications

Susceptible UTIs.

Limitations of Use

Not for treating pyelonephritis or perinephric abscesses.

Furadantin Dosage and Administration

Adult

Take with food. 50–100mg 4 times daily. Suppressive therapy: 50–100mg once daily at bedtime may suffice.

Children

Take with food. <1month: contraindicated. ≥1month: 5–7mg/kg per day in 4 divided doses; if ≥40kg, see Adult dose. Continue therapy for 1 week or for at least 3 days until urine is sterile. Suppressive therapy: 1mg/kg per day in 1–2 divided doses may suffice.

Furadantin Contraindications

Contraindications

Renal impairment (CrCl <60mL/min or significantly elevated serum creatinine), anuria, oliguria. Neonates <1 month of age. Labor & delivery. Pregnancy at term (38–42 weeks gestation). History of cholestatic jaundice or hepatic dysfunction.

Warnings/Precautions

Discontinue and treat appropriately if hypersensitivity or pulmonary reactions, hepatitis, or hemolysis occurs. Monitor LFTs periodically. Renal insufficiency. G6PD or Vit. B deficiency. Anemia. Diabetes. Electrolyte imbalance. Debilitated. Monitor renal and pulmonary function in long-term use. Nursing mothers: not recommended (monitor infants).

Renal. Half-life: 0.72–0.78 hours.

Furadantin Interactions

Interactions

Avoid magnesium trisilicate. Antagonized by, and increased toxicity with uricosurics (eg, probenecid, sulfinpyrazone). May cause false (+) glucose test with Benedict's or Fehling's solution.

Furadantin Adverse Reactions

Adverse Reactions

Nausea, emesis, anorexia, dizziness, headache, pulmonary reactions (esp. in long-term use), peripheral neuropathy, allergic reactions, C. difficile-associated diarrhea, hematologic disorders; rare: hepatic reactions, exfoliative dermatitis, Stevens-Johnson syndrome, optic neuritis, methemoglobinemia, aplastic anemia.
