Mechanism of Action

Ceftazidime is a cephalosporin antibacterial drug with in vitro activity against certain gram-negative and gram-positive bacteria. The bactericidal action of ceftazidime is mediated through binding to essential penicillin-binding proteins (PBPs). Avibactam is a non-beta-lactam beta-lactamase inhibitor that inactivates certain beta-lactamases that degrade ceftazidime. Avibactam does not decrease the activity of ceftazidime against ceftazidime-susceptible organisms.

Avycaz Indications

Indications

Susceptible infections including complicated intra-abdominal infections (cIAI), in combination with metronidazole, complicated urinary tract infections (cUTI), including pyelonephritis, and hospital-acquired or ventilator-associated bacterial pneumonia (HABP/VABP).

Avycaz Dosage and Administration

Adult

Give by IV infusion over 2hrs. ≥18yrs: 2.5g every 8hrs. cIAI (in combination with metronidazole): treat for 5–14 days. cUTI, HABP/VABP: treat for 7–14 days. Renal impairment (CrCl 31–50mL/min): 1.25g every 8hrs; (CrCl 16–30mL/min): 0.94g every 12hrs; (CrCl 6–15mL/min): 0.94g every 24hrs; (CrCl ≤5mL/min): 0.94g every 48hrs; give after hemodialysis on hemodialysis days.

Children

<31 weeks gestational age: not established. Give by IV infusion over 2hrs. ≤28 days (includes neonates postnatal age and preterm infants with corrected age) with ≥31 weeks gestational age: 25mg/kg every 8hrs; >28 days (includes full-term infants with postnatal age and preterm infants with corrected age)–<3mos: 37.5mg/kg every 8hrs; 3mos–<6mos: 50mg/kg every 8hrs; 6mos–<2yrs: 62.5mg/kg every 8hrs; 2–<18yrs: 62.5mg/kg up to max 2.5g every 8hrs. cIAI (in combination with metronidazole): treat for 5–14 days. cUTI, HABP/VABP: treat for 7–14 days. Renal impairment: ≥2yrs: (CrCl 31–50mL/min): 31.25mg/kg up to max 1.25g every 8hrs; (CrCl 16–30mL/min): 23.75mg/kg up to max 0.94g every 12hrs; (CrCl 6–15mL/min): 23.75mg/kg up to max 0.94g every 24hrs; (CrCl ≤5mL/min): 23.75mg/kg up to max 0.94g every 48hrs; give after hemodialysis on hemodialysis days.

Warnings/Precautions

Penicillin or other beta-lactam allergy. Discontinue if allergic reaction occurs. Evaluate and manage fluid, electrolyte levels, supplement protein intake if C.diff-associated diarrhea suspected or confirmed. Renal impairment; monitor CrCl at least daily and adjust dose as necessary. Pregnancy. Nursing mothers.

Concomitant probenecid: not recommended. False (+) urine glucose test possible.

Avycaz Adverse Reactions

Adverse Reactions

Diarrhea, nausea, vomiting; C.diff-associated diarrhea, hypersensitivity reactions, CNS reactions (eg, seizure, encephalopathy, coma). Children: also rash, infusion site phlebitis.
