Mechanism of Action

Rifaximin is a semi-synthetic derivative of rifampin and acts by binding to the beta-subunit of bacterial DNA-dependent RNA polymerase blocking one of the steps in transcription. This results in inhibition of bacterial protein synthesis and consequently inhibits the growth of bacteria.

Indications

Irritable bowel syndrome with diarrhea (IBS-D).

Xifaxan 550mg Dosage and Administration

Adult

≥18yrs: 550mg three times daily for 14 days. Recurrent episodes: can retreat up to two times with the same dosage regimen.

Children

<18yrs: not established.

Warnings/Precautions

Not studied in patients with MELD scores >25 (only 8.6% of patients in the controlled trial had MELD >19). Severe hepatic impairment (Child-Pugh C). Elderly. Pregnancy. Nursing mothers.

Absorption

Mean Cmax: 2.4–4 ng/mL after a single and multiple doses (550 mg).

Distribution

Plasma protein binding ratio: 67.5% (healthy patients); 62% (hepatically impaired patients).

Metabolism

Hepatic (CYP3A4).

Elimination

Fecal (96.62%), renal (0.32%). Half-life: 5.6 hours (healthy patients); 6 hours (IBS-D patients).

Interactions

Potentiated by P-gp inhibitors (eg, cyclosporine); caution. Monitor INR and PT with warfarin; dose adjustment may be needed.

Adverse Reactions

Peripheral edema, nausea, dizziness, fatigue, ascites, increased ALT.
