Mechanism of Action

Regadenoson is a low affinity agonist for the A2A adenosine receptor. Activation of the A2A adenosine receptor produces coronary vasodilation and increases coronary blood flow (CBF).

Lexiscan Indications

Indications

For radionuclide myocardial perfusion imaging (MPI) in adults unable to undergo adequate exercise stress. 

Lexiscan Dosage and Administration

Adult

Give 0.4mg (5mL) as IV inj within 10secs. Follow immediately with a 5mL saline flush post-inj. Administer the radionuclide MPI agent 10–20secs after saline flush. 

Children

Not established.

Lexiscan Contraindications

Contraindications

Sinus node dysfunction, 2nd- or 3rd-degree AV block, unless paced.

Warnings/Precautions

Have resuscitative equipment available. Avoid if acute myocardial ischemia present (eg, unstable angina, cardiovascular instability). Increased risk for serious hypotension in autonomic dysfunction, hypovolemia, left main coronary artery stenosis, stenotic valvular heart disease, pericarditis, pericardial effusions, stenotic carotid artery disease with cerebrovascular insufficiency. Bronchoconstriction. May lower seizure threshold; obtain seizure history. Pregnancy. Nursing mothers: pump/discard breast milk for 10hrs after administration.

Renal (57%). Half-life: ~2 hours.

Interactions

Avoid any products containing methylxanthines (including caffeinated coffee, tea or other caffeinated beverages, caffeine-containing drug products, aminophylline, theophylline) for ≥12hrs prior to administration. Withhold dipyridamole for ≥2 days prior to administration. 

Lexiscan Adverse Reactions

Adverse Reactions

Dyspnea, headache, flushing, chest discomfort, dizziness, angina pectoris, chest pain, nausea, abdominal discomfort, dysgeusia, feeling hot; SA/AV nodal block, atrial fibrillation/flutter, hypotension, hypertension, seizure, stroke, hypersensitivity reactions.
