Mechanism of Action

Ioflupane binds reversibly to the human dopamine transporter (DaT). Following administration in humans, radioactive decay of the iodine-123 emits gamma radiation which can be detected externally using gamma detectors, allowing visualization of the brain striata through SPECT imaging.

As an adjunct to other diagnostic evaluations for striatal dopamine transporter visualization using single photon emission computed tomography (SPECT) brain imaging in adults with suspected Parkinsonian syndromes (PS) or suspected dementia with Lewy bodies (DLB).

Datscan Dosage and Administration

Adult

Administer potassium iodide oral solution or strong iodine solution (equivalent to 100mg iodide) at least 1hr prior to dose. Give by IV inj over ≥20secs. 111–185MBq (3–5mCi). Begin SPECT imaging 3–6hrs post-inj: see full labeling.

Warnings/Precautions

Risk for radiation exposure. Should only be used by trained and experienced physicians in the safe use and handling of radionuclides to minimize radiation exposure. Ensure adequate hydration; encourage frequent voiding for first 48hrs post-administration. Have resuscitation equipment/medications readily available. Increased risk for thyroid neoplasia. Severe renal impairment. Pregnancy: exclude status prior to initiation (give thyroid blocking agent prior to inj). Nursing mothers: pump/discard breastmilk for ≥6 days after administration.

Interactions

Risk of image interference with concomitant drugs that bind to the dopamine transporter (eg, amoxapine, amphetamine, armodafinil, benztropine, bupropion, buspirone, cocaine, mazindol, methamphetamine, methylphenidate, modafinil, norephedrine, phentermine, phenylpropanolamine, selegiline, sertraline). May be potentiated or antagonized by concomitant SSRIs (eg, paroxetine, citalopram).
