Mechanism of Action

The therapeutic effects of diltiazem HCl are believed to be related to its ability to inhibit the cellular influx of calcium ions during membrane depolarization of cardiac and vascular smooth muscle. Diltiazem HCl has been shown to produce increases in exercise tolerance, probably due to its ability to reduce myocardial oxygen demand. This is accomplished via reductions in heart rate and systemic blood pressure at submaximal and maximal workloads.

Tiazac Dosage and Administration

Adult

May sprinkle contents on applesauce; do not crush or chew contents. Initially 120–180mg once daily; adjust at 1–2 week intervals; usual max 540mg/day.

Contraindications

Sick sinus syndrome, 2nd- or 3rd-degree AV block, unless paced. Severe hypotension. Acute MI and pulmonary congestion documented by X-ray on admission.

Warnings/Precautions

Heart failure. Hypotension. Impaired renal or hepatic function. Monitor liver function. Discontinue if persistent rash occurs. Pregnancy (Cat.C). Nursing mothers: not recommended.

Interactions

Caution with digoxin, β-blockers, others that may affect cardiac conduction (may lead to AV block). Monitor heart rate with concomitant clonidine. Potentiates propranolol, benzodiazepines, buspirone, cyclosporine, carbamazepine, ivabradine (avoid), simvastatin (limit simvastatin dose to 10mg/day), quinidine; consider dose adjustments; monitor. Caution with CYP3A4 substrates, inhibitors (eg, cimetidine), or inducers (eg, rifampin). Anesthetics may potentiate cardiac depression.
